Legacy Consent Entry
For paper/offline records where you already have a signature and ID files. This records a normal consent_submissions entry and generates the current release-form PDF from what you enter here — no email is sent and nothing goes through the public form. It lands in Pending Review, same as a live submission: click Archive to Drive on it afterward to log it in Drive and the Sheet.
